Our Purpose
The focus of the Diabetes Education Center is on education and self-management so that patients can improve their quality of life, stay healthier and reduce incidents of long-term complications. We assess each patient’s needs and tailor a program specifically for him or her. Our goal is to have patients commit to managing diabetes and maintaining a healthy lifestyle through self-management.
Our Team
Using a multidisciplinary approach, we bring together nurses, dietitians, pharmacists, behavior health specialists and social service workers to help patients manage their disease. Patients can also take advantage of all the resources at FMC Campus, with access to specialists in wound care, podiatry, pain management, physical and occupational therapy and more to help people thrive despite their condition.
Our Services
The Diabetes Education Center includes several key components for diabetes self-management:
- Overview of diabetes
- Medication management
- Nutrition
- Exercise
- Blood sugar monitoring
- Preventative health measures for managing acute/long term complications
- Stress management
Classes
Program includes a minimum of two classes, approximately two hours each.

Tuesday 1 pm – 3 pm
Wednesday 4 pm – 6 pm
A referral from a personal physician is required in order to participate in the Diabetes Education Center. Most health insurance plans, including Medicare, provide coverage for the cost of diabetes education. Private pay is also available.
